A new way to deliver and get paid for chronic care

The ACCESS Model (Advancing Chronic Care with Effective, Scalable Solutions) replaces traditional visit-based billing with Outcome-Aligned Payments. Instead of being paid per visit, you're rewarded for improving patient health.

50 / 50 Withhold

Half is paid monthly. The other half is released when your organization hits the Outcome Attainment Threshold — 50% of patients meeting clinical targets in Year 1.

Co-Management Pay

PCPs and referring clinicians earn ~$30 per documented care coordination review, plus a ~$10 onboarding add-on — up to ~$100 per beneficiary per year. No patient cost-sharing.

Coinsurance Waiver

Organizations may uniformly waive the 20% beneficiary co-insurance on outcome-aligned payments as an engagement incentive under a CMS safe harbor.
